ABA Therapy Enrollment 

We understand finding the right ABA Therapy provider can be challenging.  You have found us and we are here to help!  Get started in the process by following our enrollment steps below.

Step 1

We want to make sure we are the provider you have been looking for! Please fill out the pre-enrollment form and tell us about your family needs. This form helps us determine if services in your area and scheduling needs are available.

Step 2

Application

Once you know we are the right fit for you and your family, we will send you our application form to get started with the process of obtaining ABA Therapy services with us.

Step 3

Insurance Verification

After you submit your application, we will get in touch with your insurance company to verify your benefits and coverage for ABA Therapy. We will review it with you before requesting authorization to conduct an assessment and proceed with the process.

Step 4

Intake Form

Upon insurance authorization to conduct an assessment, we will send you an electronic intake packet you can fill out directly from your phone, tablet or computer. 

Step 5

Assessment

Once you complete the intake packet, we will come to your home to conduct a through assessment with your child and an interview with the primary caregiver. An individualized treatment plan will be created and submitted to insurance for authorization.

Step 6

Begin Therapy Services

After your insurance authorizes the individualized treatment plan and service hours, your child may begin receiving ABA Therapy Services!
